Given the enormous progress in the treatment of diseases and the increase in survival rates, the pediatric world is also undergoing substantial change, with a significant rise in the number of children with chronic and/or complex diseases who require new models of care. The National Health Service (NHS) is therefore directing its efforts accordingly. Diagnostic, therapeutic, and care pathways (PDTA), integrated clinical networks, home care, and intermediate care represent the pillars on which this innovative approach must be built — all while strengthening hospital – community integration. The university hospital “Azienda Ospedaliera Universitaria Meyer” (AOU Meyer), with its 250 multispecialty beds, is at the center of this process, creating synergies with all stakeholders involved in the care of these children. Family involvement in integrated care is the cornerstone of this pathway. Care and education go hand in hand: ensuring educational continuity and addressing care, psychological, and environmental needs are inseparable within a context of ‘global care’.
